SMS Text Messaging Consent and Communications

Trinity Insulation LLC uses SMS text messaging to send operational

communications to our drivers, employees, and contractors. This section

describes how we collect consent for these messages, what types of

messages we send, and how recipients can manage their participation.

How We Collect Consent

Consent to receive SMS text messages from Trinity Insulation LLC is voluntary. Providing this consent is NOT a condition of employment, contracting, hiring, continued employment, compensation, or work assignments. Individuals may decline SMS consent, or opt out at any time after consenting, without any negative impact on their employment, contractor status, or business relationship with Trinity Insulation LLC. Individuals who do not consent to SMS will receive the same operational communications by phone call, email, or in person.

Consent is collected in writing during employee and contractor onboarding through a stand-alone, optional SMS Consent Form. The form is separate from required onboarding documentation (I-9, W-4, employment agreement, and other required paperwork) and is clearly labeled "Optional — Not Required for Employment." The form requires the individual to provide their mobile phone number and sign a written acknowledgment stating:

"Consent to receive SMS text messages is voluntary and is NOT a condition of employment or contracting with Trinity Insulation LLC. You may decline this consent or opt out at any time without any impact on your employment, contractor status, or business relationship. By providing my mobile phone number and signing below, I voluntarily consent to receive operational SMS text messages from Trinity Insulation LLC, including daily delivery route assignments, dispatch updates, and scheduling notifications. I understand that message frequency varies based on daily dispatch schedule, that message and data rates may apply, that I may opt out at any time by replying STOP, and that I may reply HELP for assistance."

The signed form is retained in the company's HR records, separately from required employment documentation, and is available upon request for verification purposes.

Types of Messages We Send

Operational SMS messages from Trinity Insulation LLC include, but are

not limited to:

- Daily delivery route assignments and dispatch instructions

- Vehicle, driver, and warehouse scheduling updates

- Time-sensitive changes to delivery schedules or job sites

- Internal operational communications related to an individual's

assigned work

Trinity Insulation LLC does not use SMS messaging for marketing,

promotional offers, or third-party advertising. SMS messages are sent

only to individuals who have provided prior written consent through

the onboarding process described above.

Message Frequency and Cost

Message frequency varies based on the daily dispatch schedule and the

individual's active work assignments. Message and data rates may apply

depending on the recipient's mobile carrier and plan.

Opt-Out and Help

Recipients may opt out of receiving SMS messages at any time by

replying STOP to any message received from Trinity Insulation LLC.

Upon receiving a STOP reply, the recipient will be unsubscribed from

further SMS messages and will receive a confirmation of their opt-out.

Recipients may reply HELP to any message to receive contact information

and assistance regarding the SMS program.

Recipients who wish to re-subscribe after opting out may contact

Trinity Insulation LLC directly or complete a new consent form.

Data Sharing

Mobile phone numbers and SMS consent information collected by Trinity

Insulation LLC are used solely for the operational purposes described

above. We do not sell, rent, or share mobile phone numbers with third

parties for marketing purposes. Mobile information is shared only with

Trinity Insulation LLC's SMS service provider (Twilio) solely for the

purpose of delivering operational messages, as described in this

Privacy Policy.
